| Description Lot No. 1 Repair and Maintenance Services 1 Preventive Operation Maintenance of Sewage Treatment Plants of the PGBh Hospitals – Quarterly Preventive Maintenance Services for One (1) STP Qty: 1 Lot One STP Technician to perform Preventive quarterly maintemance services of the Sewage Treatment plant including chemicals for 2 quarters, (within July to Dec. 2025) with the following scope of work: A.1 Replace Oil, Grease and Lubricants A.2 Reallignment of pulley of blower, replacement of fan belt (if necessary) A.3 Check and perform preventive maintenance on other equipment A.4 Check Control Panels (Amperes, voltage reading, retightening of bolts and cleaning of panels Enclusions: Equipment repair, hauling overhauling unit and major component replacement and upgrading, bearings mechanical seals, shafting other materials and control accessories. Supply of Chemicals and Test Kit for One (1) STP B.1 Supply of 20kg Chlorine Hypochlorite B.2 Polyaluminum Chloride (PAC) 12.5 KG B.3 Supply of test kits (ammonia, nitrate,phosphate pH & Chlorine) Quarterly Laboratory Testing and Quarterly PM Report a.
